Occupational Therapist - Community Stroke Rehab

Position Overview

Working to deliver personalized rehabilitation services and support patients in achieving their goals for recovery and independence.

Position Responsibilities

  • Patient Assessment: Conduct comprehensive assessments of patients’ physical, cognitive, and emotional abilities related to their stroke recovery, including home and community environments.
  • Therapeutic Interventions: Develop and implement individualized treatment plans focusing on improving patients’ activities of daily living (ADLs), fine motor skills, cognitive functioning, and adaptive techniques.
  • Rehabilitation Strategies: Provide a range of occupational therapy services, including therapeutic exercises, functional training, assistive device recommendations, and patient and caregiver education.
  • Collaborative Care: Work collaboratively with physicians, physiotherapists, speech-language pathologists, and other healthcare professionals to ensure a coordinated approach to patient care.
  • Patient & Family Support: Offer education and guidance to patients and their families on stroke recovery processes, strategies for managing daily activities, and the use of adaptive equipment.
  • Documentation & Reporting: Maintain accurate and detailed records of patient assessments, progress notes, and treatment plans in accordance with hospital policies and professional standards.
  • Community Engagement: Participate in community outreach programs and educational initiatives aimed at increasing awareness of stroke prevention, rehabilitation, and available services.

Position Requirements

  • Must have a degree in Occupational Therapy from an accredited institution; a Master’s in Occupational Therapy is preferred.
  • Current registration with the College of Occupational Therapists of Ontario (COTO) and a valid license to practice occupational therapy in Ontario.
  • Minimum of 2 years of clinical experience in a rehabilitation setting, with specific experience in stroke rehabilitation preferred.
  • Proficient in assessing and addressing the functional impacts of stroke on patients.
  • Excellent interpersonal and communication skills.
  • Experience with electronic medical record systems.
  • Ability to create and implement evidence-based therapy plans.
  • Patient-centered and empathetic approach to care.
  • All TADH staff members must follow the COVID-19 vaccination series in accordance with the most up-to-date vaccine guidance provided by the Ontario Ministry of Health.

Compensation & Benefits

  • Starting salary of $40.36/hour – $78,702.00/year (as per step 1 of the OPSEU collective agreement).
  • Option to enroll in the Healthcare of Ontario Pension Plan.
