Position Overview

The post-crisis social worker will follow up with clients who are accessing or have accessed Crisis services at TADH. Their role is to ensure referrals for services have been completed and to provide a warm hand-off to agencies being referred to. Crisis services include MCRRT interactions, ED Crisis, OTN Crisis and patients discharged from the Acute In-Patient Mental Health Unit.

Position Responsibilities

  • To provide follow up post-acute crisis assessment and intervention.
  • Effectively function within the framework of The Ontario College of Social Workers and Social Service Workers’ professional standards.
  • To work effectively with community agencies.
  • Conduct interviews with clients or families in crisis.
  • Coordinate with other community agencies in the provision of necessary services.
  • To inform community agencies of any changes that may affect them.
  • Inform our community of resources available to benefit those in crisis.
  • Provide a client focused philosophy in all interactions.
  • Able to implement strategies that effectively facilitate change.
  • Demonstrate leadership skills, sound judgment and diplomacy.
  • Strong assessment skills and ability to coordinate services.
  • Comprehensive knowledge of community mental health and addiction networks.
  • Strong problem solving and critical thinking skills.
  • Computer literacy with developed skills in various software applications including MS Office, RAI, ESP.
  • Able to manage high volumes and varied work assignments with accuracy.
  • Knowledge and sensitivity to work in a culturally diverse environment.
  • Demonstrate understanding and comply with client and staff safety policies.

Position Requirements

  • Baccalaureate in Social Work from a recognized university may be considered, M.S.W. preferred.
  • Must be certified with the Ontario College of Certified Social Workers and Social Service Workers.
  • Minimum of 3 years recent acute Mental Health and Addictions experience.
  • Working knowledge of impatient and community-based resources for children and adults with mental health and addictions needs.
  • Must have the ability to function as a member of the multi-disciplinary health care team.
  • Must possess a thorough knowledge of the principles and techniques associated with Crisis Intervention assessment.
  • Excellent verbal and written communication skills.
  • Must possess computer skills and knowledge of Meditech and an ability to collect relevant data and reports.
  • Employment history which demonstrates regular attendance at work.
  • You must be available to work all shifts including weekends.
  • Must possess the ability to provide effective services to clients in both English and French. Individuals who do not meet the language requirements of this post are invited to submit their résumé for future consideration.
