Social Worker – System Navigator

Position Overview

Primarily working within the Emergency Department, provide assessment, advocacy, referrals, and navigation to support individuals and families presenting to the Emergency Department with substance use and/or concurrent disorders. The successful candidate is expected to work with the community withdrawal management team, the addictions medicine consult service, and community partners to develop a holistic plan of care appropriate to their presentation.  Work within the addictions services umbrella to provide support to other addictions programs as required.

Position Responsibilities

  • Provide assessment, referrals, and navigation to support individuals and families presenting to the Emergency Department with substance use and/or concurrent disorders.
  • Provide individual and group care, including but not limited to crisis management, counseling, psychoeducation, connection to community-based resources for patients and their supports in partnership with the Addiction Medicine Consult Service.
  • Provision of strength-based interventions that build off the inherent worth of people seeking support with a substance use disorder.
  • Works in collaboration and consultation within the multidisciplinary team meet the patient’s needs.
  • Facilitates strong working relationships and acts as a resource within our internal Mental Health and Addiction programs, other areas of TADH, as well as with community partners.
  • Document assessments, intervention plans, progress notes and outcomes in a timely manner.
  • Practice from an anti-oppressive, culturally safe, harm reduction approach and provide health teaching to clients, families and/or significant others on typical recovery and lifestyle issues throughout all stages of recovery.
  • Facilitate formal care paths for ongoing care in the district including connections to RAAM, withdrawal management services, and substance use treatment.
  • Participate in program development at the local and regional level.
  • Capture relevant and required data as defined by the Ontario Health, HQO, Accreditation or other quality improvement strategies.
  • Acts as a patient/family advocate. Responds to patient/family concerns and escalates to Management as required.
  • Align improvement projects with the organization and monitor or adjust to achieve greater outcomes.
  • Educate and promote health, safety and wellness in the workplace.
  • Evaluate services and satisfaction on an on-going basis and formulate improvement strategies and solutions.
  • Assists with coverage in all MH&A programs as required.
  • Perform other duties as required.

Position Requirements

  • Minimum of a Bachelor’s Degree in Social Work from an Accredited University.
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers is required.
  • Minimum of 3 years’ experience and knowledge of addictions related care (i.e. withdrawal symptoms, withdrawal management and medication treatment available).
  • Ability to use tact and discretion in dealing with health care providers and patients.
  • Experience in program development and education.
  • Certificate in Harm Reduction would be considered an asset.
  • Demonstrated health care knowledge of psychiatric conditions.
  • Demonstrated knowledge of evidence based practices of recovery principles and trauma informed care.
  • Demonstrated knowledge of how to apply harm reduction approach to recovery.
  • Understanding and knowledge of the recovery oriented approach.
  • Demonstrated experience dealing with clients who have poor distress tolerance and emotion regulation challenges.
  • Demonstrated ability to work with digital technology.
  • Demonstrated critical thinking skills.
  • Effective time management and ability to prioritize workload.

Compensation & Benefits

  • Starting salary of $39.16/hour; $76,362.00/year (As per Step 1 of the OPSEU collective agreement.) range to

50.77/hour; $99,001.50/year (As per Step 6 of the OPSEU collective agreement.)
