We ask that you provide:
- Whether or not your feedback is a compliment or concern.
- Where you or your loved one received care.
- The date and times you attended TADH.
- The names of the people your feedback is about.
- Contact information so that a member of our Patient Relations team can contact you.
Absolutely! We ask that you please provide as many details about your visit as possible, including where you or your loved one received care. With your consent, our Patient Relations team will review your, or your loved one’s, electronic medical record as part of the investigation process.
Yes, our Patient Relations team reviews all submitted feedback thoroughly. If you are not the patient, or substitute decision maker (SDM), we may be limited in what information we can share with you without the consent of the patient or SDM.
- A member of our Patient Relations team will be in contact with you within 3 business days.
- If you are submitting a concern, a formal review will be initiated.
- We will facilitate communication with any relevant departments or services to help find a resolution.
- A response to your concern, including an explanation of any decisions or actions taken as a result.
All individuals have the option of contacting the Patient Ombudsman, an entity who oversees the complaint process for public hospitals, long-term care homes, and home and community care support services, including Ontario Health at Home. The provincial Patient Ombudsman can be contacted by:
