Geriatric Assessor - In-patient Geriatric Consultation Service

Position Overview

To participate in the Inter-Disciplinary Geriatric in Patient Consult Team to provide expert consultation and treatment to patients over the age of 65, centered in comprehensive geriatric assessment.

Position Responsibilities

  • This team functions in an interprofessional model where team members act as Geriatric Assessors, sharing a common set of competencies.
  • To continually evaluate the client’s treatment program and adjust the treatment program so that it meets the client’s goals.
  • Applying best practices for dementia, delirium depression and other mental health issues and their effect on patients, families and service providers.
  • Applied skills in acute management of geriatric syndromes, including delirium, polypharmacy, falls and functional decline, acute/sub-acute cognitive changes, bowels/bladder, and nutrition/hydration problems.
  • Applied knowledge of chronic disease management in the older population.
  • Supporting allied health responsibilities within clinical team such as discharge planning, coordination of care, family meetings, assessments/treatments deemed acceptable within respective regulatory college.

Position Requirements

  • Must have a minimum of 4 years Bachelor degree or Masters Degree in health related field from an accredited university (for example; Bachelor of Social Work, Masters of Science and Physiotherapy, Masters of Speech Language Pathology, Masters of Occupational Therapy or other equivalent)
  • Current registration in good standing with relevant regulatory College.
  • Must possess the verbal ability to communicate effectively with clients, families and other professional personnel.
  • Demonstrated person and caregiver-focused care, supported by evidence based clinical practices.
  • Must possess the ability to work with minimal supervision and must follow standardized procedures.  Should be adaptable to cope with changing contacts with people, situations and conditions.
  • Must have employment history, which demonstrates regular attendance at work.
  • Bilingualism (French and English) would be considered an asset.
  • All TADH staff members must follow the COVID-19 vaccination series in accordance with the most up-to-date vaccine guidance provided by the Ontario Ministry of Health.

Compensation & Benefits

  • Rate of Pay: To be determined, based on experience and certification.
  • Option to enroll into Healthcare of Ontario Pension Plan.
