Position Overview
To participate in the Inter-Disciplinary Geriatric in Patient Consult Team to provide expert consultation and treatment to patients over the age of 65, centered in comprehensive geriatric assessment.
Position Responsibilities
This team functions in an interprofessional model where team members act as Geriatric Assessors, sharing a common set of competencies.
To continually evaluate the client’s treatment program and adjust the treatment program so that it meets the client’s goals.
Applying best practices for dementia, delirium depression and other mental health issues and their effect on patients, families and service providers.
Applied skills in acute management of geriatric syndromes, including delirium, polypharmacy, falls and functional decline, acute/sub-acute cognitive changes, bowels/bladder, and nutrition/hydration problems.
Applied knowledge of chronic disease management in the older population.
Supporting allied health responsibilities within clinical team such as discharge planning, coordination of care, family meetings, assessments/treatments deemed acceptable within respective regulatory college.
Position Requirements
Must have a minimum of 4 years Bachelor degree or Masters Degree in health related field from an accredited university (for example; Bachelor of Social Work, Masters of Science and Physiotherapy, Masters of Speech Language Pathology, Masters of Occupational Therapy or other equivalent)
Current registration in good standing with relevant regulatory College.
Must possess the verbal ability to communicate effectively with clients, families and other professional personnel.
Demonstrated person and caregiver-focused care, supported by evidence based clinical practices.
Must possess the ability to work with minimal supervision and must follow standardized procedures. Should be adaptable to cope with changing contacts with people, situations and conditions.
Must have employment history, which demonstrates regular attendance at work.
Bilingualism (French and English) would be considered an asset.

Compensation & Benefits
Rate of Pay: To be determined, based on experience and certification.
Option to enroll into Healthcare of Ontario Pension Plan.
