The Complex Continuing Care Unit (CCC) provides care for patients with chronic, complex, and multiple non-urgent medical needs who need to stay in hospital for specialized services. This inpatient unit supports patients before they transition to their destination of home, long-term, or community care.

A highly specialized team of physicians, nurses, physiotherapists, occupational therapists, rehabilitation assistants, social workers, speech-language pathologists, dietitians and pharmacists, supports the CCC Unit. Together, our team creates an environment that is patient and family centered, and senior friendly.


Complex Continuing Care services
  • Complex care for patients with chronic conditions or multi-system disease.
  • Short-term inpatient rehabilitation for patients who may transition back home or to a long-term care, or community care setting.
  • System navigation and discharge planning.
