Your Rights as a Patient

You have the right to:
  1. Receive safe, high-quality care from qualified staff.
  2. Know the name, role, and duties of the people involved in your care.
  3. Have your privacy respected and your personal information kept confidential.
  4. Ask for a different care provider if there’s a situation or personal conflict that may affect your care.
  5. See your health record upon request, which will be kept accurate and up to date.
  6. Be treated with kindness, respect, and fairness — free from abuse, neglect, or discrimination.
  7. Receive care that respects your values, culture, and spiritual beliefs.
  8. Take part in decisions about your care and make your own choices.
  9. Receive care in an environment that values and learns from the experiences of patients, families, and caregivers.
  10. Get clear information to help you and your support person understand your health and treatment options.
  11. Be informed about any costs that aren’t covered by the Ontario Health Insurance Plan (OHIP).
  12. Refuse a treatment and be told what might happen if you do.
  13. Ask for a review of care decisions, make a complaint, or share feedback about your care.
  14. Choose whether or not to participate in research or clinical trials.

Your Responsibilities as a Patient
  1. Treating other patients, visitors, and health care staff with respect – no violence, threats, or abusive behaviour.
  2. Giving accurate and complete information to help plan your care.
  3. Reporting any safety concerns or risks you notice.
  4. Following hospital rules and policies.
  5. Understanding that TADH is not responsible if personal items or assistive devices are lost, stolen, or damaged.
  6. Getting approval from a manager or physician before taking photos, videos, or audio recordings to protect everyone’s privacy.
  7. No smoking or vaping on hospital property.
