Admitting / Registration Department

Admitting & Registration is the first step to receiving care at our hospital. Admitting & Registration is responsible for collecting and confirming personal health information. This information is shared with your health care team to ensure safe, quality patient care.

During Registration

Registration is an important step in your care, and your information must be checked each time you visit the hospital. In a process called positive patient identification, our staff will verify your identification when you register. They will also ask you infection control screening questions.

Location

The Admitting & Registration desks are located on the main level and can be assessed from the main promenade front doors or through the emergency department waiting room.

Hours of Operation

The Admitting/Registration department is staffed 24 hours/day, 7 days/week and provides registration services for over ? emergency and ambulatory care patients attending a multitude of hospital services. We maintain an accurate hospital bed census by admitting, transferring and discharging over ? inpatients to their appropriate service and accommodations. The staff also assists in the Switchboard desks and Health Records department as required.

When you arrive at the Hospital to be seen in the Emergency department, please go directly to the Admitting/Registration desks to register.

The first person you meet at the Admitting/Registration department is the Information Services Clerk. To register with the Information Services Clerk you will need your health card (OHIP card). If you do not have an OHIP card, please bring another form of government-issued photo ID (such as a driver’s license, passport, or other provincial health card).

What to bring - Documentation

  • Your Ontario health card.  If you do not have a health card, please bring government-issued photo identification. Such as a driver's license, passport, or permanent resident card.
  • Other insurance information if you do not have an Ontario Health card such as Out-of-Province Health insurance or Interim Federal Health Program. 
  • Information about private health insurance or other health benefits such as Your insurance policy company name, address and phone number
  • The names, addresses and telephone numbers of your family members, friends or other emergency contacts.
  • The name, address and telephone number of your family physician.
  • A credit card number for any services that your insurance doesn’t cover (you and your insurance company will receive a bill and the credit card will be charged only with your consent).
  • Workplace Safety and Insurance Board (WSIB) file number and information about your accident (if you’re in the hospital due to an accident at work)
  • If you live outside of Canada, please bring your identification such as your passport, private health insurance or other health benefits documents.

What to Expect

Once registered, you will receive an armband with your name and personal health information on it. The armband will be checked throughout your visit to ensure safe, quality patient care.

Fees

As a patient, you may incur costs not covered by OHIP. Costs may include charges for accommodation, transportation, supplies or equipment. These costs are the responsibility of the patient and their family. 

If you live in Ontario and have a valid health card, you are covered by the Ontario Health Insurance Plan (OHIP) and are able to receive standard care.

Additional fees are charged for items such as:

  • Preferred room accommodation
  • Ambulance
  • Delisted procedures (a procedure that is no longer covered by OHIP)
  • Uninsured procedures (a procedure that is not covered by OHIP)
  • Medical devices
  • Autopsies
  • Complex Care Co-payments (CCC) for room and accommodation
  • Release of information for copies of your medical chart
  • Out of province patients may incur an additional fee to be paid upfront for services

Private Medical Coverage & Health Insurance

The Admitting/Registration department is unable to check or confirm eligibility for private medical coverage or health insurance. Please have this information with you before coming to the hospital.

Accommodation

We will make every effort to provide the type of room you prefer, but the medical needs of other patients can affect our ability to fulfill your request. We appreciate your understanding.

Medications

All current medications in their original bottles. 

A complete list of medications you are currently taking. Including, prescription medications and non-prescription medications. As well as, vitamin or mineral supplements and herbal remedies.
